If you are a parent or teacher and get an error message, please confirm that you are entering the correct information. The child’s name, DOB, Case # and Clinic ID must exactly match the assignment slip. If you still get an error message, contact the child’s clinic. They need to verify you have the correct information on the rating request slip. If you have verified your data entry and an error message still appears, the clinic should check that they entered valid information (name, DOB) when they initially registered the patient.
